Fees, Insurance, and Cancellation Policy

Queenstone Fire Road

Fees

For individual therapy, my session fee is $275 for a 50-minute session. Couples therapy sessions are 75 minutes in duration, and the fee for each session is $350. I understand the financial considerations of different individuals and offer a sliding scale for active duty personnel, Veterans, and first responders.

Given my extensive clinical experience of over 30 years, specialized training, expertise, and relevant research and teaching background, my rates are in line with other psychologists in the Bay Area who possess similar depth and breadth of training and experience.

Payment is required in full at the beginning of each appointment.  I accept

Insurance

I am an out-of-network provider. If you have a preferred provider organization (PPO) insurance plan and would like to use your insurance for therapy sessions, upon your request, I can provide you with a monthly invoice or "Superbill." This document will include all the necessary information, such as dates of service, ICD-10 Diagnostic Code, CPT Procedure Code, and Corporation Tax ID number. You can submit the Superbill along with a claim form (which can usually be downloaded from your insurance carrier's website) to your insurance carrier for direct reimbursement. The amount of reimbursement will depend on your insurance coverage, as it varies by carrier and plan.

Before our first session, I recommend contacting your insurance carrier to inquire about reimbursement for sessions with an out-of-network psychologist. Here are some helpful questions to ask your insurance carrier regarding your therapy coverage:

  • Does my plan provide out-of-network reimbursement for mental health services (with treatment code 90791 for the initial visit and 90834 for subsequent visits)?

  • How many sessions are covered per calendar year?

  • What is the amount or percentage reimbursed for out-of-network mental health therapists (specifically for clinical psychologists with a PhD)?

  • Is there a maximum allowable amount per session or a cap on reimbursement?

  • What is my deductible, and has it been met?

Cancellation policy

I maintain a 24-hour cancellation policy. If you cancel your scheduled appointment or fail to show up within 24 hours of the appointment time, I reserve the right to charge you for the missed session. Please note that insurance providers typically do not cover missed appointments or last-minute cancellations.
